Student Athletic Health Forms

 

 
 

Midland Park students will need the following paperwork which is mandated by the state to participate in athletics at the beginning of each sport season:

Physical forms:
 
  1. Physical Form (Parts A& B) Both forms must be completed by athlete’s physician in order to participate in a sport.  The forms will then be reviewed and cleared by school physician (including the eye screening).  These forms only need to be submitted once every 365 days.                                                                                                                                                                                                             (or)

 

  1. Physical Update- This form is utilized if the physical is more than 60 days old.  If there is no change in health, the nurse may approve it.  If there has been a medical issue, then a doctor’s clearance will be needed to participate and then cleared by school physician.

 

 

Consent Forms:
 

(All forms require the student’s and parent’s signature)

 

  1. Parental Consent Form- By signing this form, the parent or guardian provides consent for their son/daughter to participate in a sport.    

 

  1. Steriod Consent Form- By signing this form, the parent or guardian is giving consent that a random drug test may be conducted on their son/daughter in accordance with the NJSIAA steroid testing policy. 

 

  1. Concussion Form- By signing this form, the parent or guardian has read and understands the signs and symptoms of a concussion.  The parent or guardian must read and understand the Midland Park Concussion Policy and “Return to Play” procedure.
 
 
* All physical forms and consent forms are to be returned to the school nurse.
Paperwork should not be submitted to the Athletic Director, or any coach or trainer.
